If you were a patient of Jefferson Healthcare between March 19, 2020, and March 19, 2024, and visited the Jefferson Healthcare website (http://jeffersonhealthcare.org) between March 19, 2020, and March 19, 2024, and the visit to the Jefferson Healthcare website was for purposes related to the provision of your own healthcare, then you may be a Settlement Class Member.
Excluded from the Settlement Class are: (1) all persons who timely and validly request exclusion from the Settlement Class; (2) the Judge assigned to evaluate the fairness of this settlement and the Judge’s staff and immediate family members (3) officers and directors of Jefferson Healthcare and (4) the legal representatives, successors or assigns of any such excluded person.
If you are a Settlement Class Member and wish to receive a voucher you must complete a claim form online or postmarked on or before November 17, 2025.
Defendants have agreed to purchase vouchers for a twelve (12) month subscription for the CyEx Privacy Shield product. The CyEx Privacy Shield product features include Data Broker Opt Out, VPN in Touch, Digital Vault, Password Defense, Dark Web Watchlist, Password Scan, and Private Search.You may be entitled to receive a voucher if you are a valid Settlement Class Member and file a valid, timely Claim Form.

To start your online Claim Form, you can find your Unique ID and PIN on the Notice that was sent to you. Please request your Unique ID & PIN by providing your full name and mailing address.
